forms of payment

We accept all major credit cards, cash, and FSA and HSA (Federal and Health Savings Account) Cards.  

insurance

We are in-network with Blue Cross Blue Shield and collect deductibles, co-pays, and co-insurance at each visit.

For all other insurance carriers, payment in full is due at the time of service. We are happy to submit to your insurance as an out of network provider if your plan covers chiropractic or acupuncture AND you have a PPO plan.   If you would like to know if your health insurance plan covers chiropractic or acupuncture, please call your insurance provider.

cancellation policy

We require at least 48 hour notice to cancel an appointment. Call or text us at 847. 461. 9445 or send an email to info@nurturewomenswellness.com.  If you cancel with less than 48 hours until your scheduled appointment, you will be billed in full for your visit.  We appreciate your understanding.

 

arriving to your appointment

We do our best to run on time. We are a small practice, providing one-on-one care; we respect your time, please respect ours. We want you to receive the full benefit of your appointment time and if you are running late, it limits the amount of time we have for questions, therapy, or relaxing acupuncture sleep. 

if you are more than 15 minutes late, we might not be able to treat you and you will be charged in full for your appointment. 

New Patients: Please fill out your online intake paperwork prior to your appointment.

Follow-up patients: Please arrive at least 5 minutes prior to your scheduled appointment time so that you may use the bathroom or relax for a few minutes before your appointment.
